I've always thought it was a timing idler type pulley just making noise. I did a timing belt kit replacement w/water pump at 160k miles and I now have 215k miles, that's ~60k miles on the components. I used to jump at any noise and thru the years I've come to worry less about some things, this being one in that I hope its just a nosily bearing.
